Logic Model is a program planning and evaluation tool that illustrates the logical linkage between the program’s problem, activities and the intended impact. The Logic Model Framework works as your program roadmap with essential information for the stages of planning, communication, implementation and evaluation. Use the Evaluation Logic Model Slide, Logic Model Flow Diagram, and the Logic Model Pyramid to align the elements of your project or program to the desired outcome.
A basic Logic Model Framework follows the input-output-outcome format, which technically covers what you need to deliver a successful project. However, we decided to upgrade this visual diagram with three boxes to showcase outcomes indicators for the short-term, medium-term, and long-term. The result is a complete planning and evaluation tool.
